All intake appointments cost $50.00. There is a $25.00 minimum payment in order to fill out the intake paperwork. The rest of the intake fee can be paid at the counselor meeting.

Before we can schedule you for an intake appointment, you will need to come into our Main Office (1591 Chambers Road, Suite E) to fill out the intake paperwork. The intake paperwork consists of documents that are required by the state, as well as documents needed to insure you receive the best possible treatment for your individual needs. Please view our intake hours to see when you can come in to complete this paperwork. The intake paperwork takes approximately one hour to complete. You do not need to schedule ahead to fill out paperwork. We are unable to provide intake paperwork outside of our office, and in order to maintain your safety and confidentiality, the paperwork must be completed in our office.
Once the intake paperwork has been completed, we will schedule you to meet with a counselor to finish the intake appointment. During this meeting, the counselor will go over the paperwork with you, explain our agency policies, and get you scheduled into group or individual sessions as needed.
